I tend to download most of my resources in bulk or at the very least, add the credit in bulk. For that I've got an extra download folder. I usually credit (by foldername(= username for IJ/LJ/DA/etc)) before unpacking everything there is to unpack and sorting it into the "real folders" (aka where they need to be so that I can use them)... so even if credit isn't given immediately, it will be before I start using the textures/brushes.
If you made some of the textures/brushes and the name/community to credit has changed and want me to update this list to reflect this: just let me know through a comment here.
In addition to the resources listed here, I have added gradients, brushes and other things from the German version of Photoshop Creative to my Photoshop. I will add these later on, when I have more time to figure out what kind of credit is needed there.

While I don't remember a lot of it by now aside from what I use here on dreamwidth or other places allowing some html code, I had a class at university that covered html, css and other web-related coding and it's given me some good starting points.
I copied the code from the three column table (minus the giant list of people I credited) into the textarea below in case you want to play around with it yourself. The colspan attribute allows you to define that a cell should cover more than one column and I used that to create the headers for each table and then I just played around via previews / continuously editing a post to see where all the people/places I needed to credit would be best split between the columns so they would be roughly evenly filled.
And leaving an empty line every time I went to the next letter in the alphabet because I had a hard time finding out where I had to add whoever needed to be added. And I started doing it alphabetically, because I lost track of if I was already crediting someone or not.
